Abstract
Complete or partial airway obstruction is a recognised complication of tracheostomy.1 Sixty minutes after a surgical tracheostomy a 66-year-old patient developed signs of acute bronchospasm. Tidal volumes and oxygen saturations fell. Bedside fibre-optic bronchoscopy showed the tip of the tracheostomy tube was partially occluded by the tracheal wall. The scope passed freely beyond the apparent obstruction and the carina was visualised. The tip of the tracheostomy tube was thought to be abutting the posterior tracheal wall and so the adjustable flange was temporarily loosened and the tube advanced into the trachea.
