Abstract
As the most frequent endocrine disorder in women of reproductive age, polycystic ovary syndrome (PCOS) poses a significant disease burden in this population. In addition, PCOS exhibits a close association with multiple cardiometabolic diseases, including obesity (particularly central), insulin resistance, and type 2 diabetes. Obstructive sleep apnea (OSA) constitutes another prevalent chronic condition, which is characterized by recurrent hypopnea/apnea episodes during sleep and also exhibits significant overlap with obesity and the metabolic syndrome. As such, PCOS and OSA are frequently observed together and in combination with other metabolic syndrome manifestations, with central obesity and insulin resistance typically constituting the common denominators that drive the underlying pathophysiology. Notably, OSA often remains underdiagnosed, particularly in women, until severe symptoms become apparent. In this context, existing clinical guidelines on the management of PCOS recommend screening for OSA in women with PCOS and increased body mass index, particularly in order to promptly identify and alleviate OSA-related symptoms. However, there is still a relatively limited number of studies on the potential interactions between PCOS and OSA and on the pathophysiologic mechanisms that link these two conditions. The present chapter presents a synthetic overview of the pertinent literature on the associations between PCOS and OSA, focusing on the relevant epidemiological evidence, key interactions, and the underlying pathophysiology.
