Uncontrolled hemorrhage resulting in hemorrhagic shock is a major cause of death following trauma. Current management of external hemorrhage in out-of-hospital care in the United Kingdom is with the use of field dressings, direct pressure, elevation, pressure on proximal artery and/or a tourniquet if exsanguinating. There are increasing calls for the use of new hemostatic agents for the management of external hemorrhage in out-of-hospital care. However, research using animal models is mixed with some studies reporting a decrease in bleeding following the application of some types of hemostatic agents, whilst other studies found no difference compared to direct pressure. It is argued that differences in outcome are related to product usage and injury type and therefore some types of agent are more effective with certain types of injury. The use of hemostatic agents for trauma injury in clinical practice have mostly been undertaken in the military setting and therefore the research supporting their use in the civilian clinical setting is inconclusive. Although some studies suggest a possible benefit for the use of some types of hemostatic agents it is important to review the current evidence to determine the applicability of the use of hemostatic agents in the management of external hemorrhage in civilian out-of-hospital.
