Abstract
Current difficulties in recruiting general practitioners (GPs) are well documented. At a time when many GPs recruited from overseas in the 1960s are about to retire, less than a third of medical students show an interest in primary care. The overall number of GPs entering the NHS exceeds the number leaving, but the composition of the workforce has changed and fewer qualified GPs are taking up full-time principal posts.
In 2000, the Royal College of General Practitioners estimated that an addi- tional 10 000 full-time GPs were needed to implement The NHS Plan Proposed initiatives to support this expansion included improved postgraduate training, and hybrid posts tailored to applicants' aspirations. The latter were to include salaried posts that could offer greater flexibility than traditional partnerships.
Salaried GP posts were made possible by the NHS (Primary Care) Act 1997. The intention was to use salaried schemes within Personal Medical Services (PMS) pilots to improve GP recruitment and retention, especially in underdoctored areas. Posts were to be used to enhance service provision in key areas and fill specific gaps in services, such as care for the homeless.
